Construction Inspection - E&D Class 2 This inspection was conducted with the on site superintendent. A PDF copy of this inspection report was emailed to Tylo D.. at the tie of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the following prior to re-inspection scheduled for 11/29/2022; 1. Finish installing the dedicated hand sink on the north side of the front service area so that hand sinks are visible to all work stations. 2. Provide a minimum of a semi-gloss paint finish to all exposed bare wood including the underside of countertops and inside the cabinets.. 3. Finish installing the permanent sneeze guard along the exposed portions of the front prep area. 4. Provide a metal cove base finish at the exposed, floor/wall junctures on the outside of the walk-in refrigerator. Also, cover and seal the spaces between the walk in unit and the surrounding walls with metal flashing. 5. Provide a stainless steel side splash between the food prep sink and the adjacent three compartment sink. 6. Provide a semi-gloss paint finish to all drywall areas throughout the food prep and food storage areas including the front area, the back room where the walk-in unit is located on the pass through and behind and above the front counters. This includes the bottom of the soffit above the front counters. 7. Ensure that the restroom door is self-closing and finish the drywall in the restroom with a minimum of a semi-gloss paint finish. 8. Provide an exhaust fan for the restroom. 9. Mop sink faucets cannot be used to feed chemical dispensers. No county legal action will result from this inspection.
