Observed a bucket of raw chicken and chicken juices on cart outside of walk-in at 51-60*F. Manager stated chicken had been placed there to thaw overnight. Manager voluntarily discarded chicken. TCS cold held foods must be held at or below 41*F to prevent growth of bacteria.

Core-3-304.12 , C: In-Use Utensils, Between-Use Storage Observed rice spatula to be stored in standing water observed to be at 70*F. Manager replaced with clean spatula in a dry container. Utensils must be stored dry, above 135*F, or under running water to prevent growth of bacteria.

Core-4-601.11, C: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Observed metal racks, corner of floor, and unit fan in walk-in fridge to have an accumulation of black and white organic matter resembling mold. A mold-like smell was also observed in unit. Unit must be deep cleaned or equipment replaces to prevent possible contamination of food.

Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods Observed ceiling in women's restroom to have what looks like water damage and is beginning to break off from surrounding plaster. Observed a broken ceiling tile by bathrooms and a pipe appears to be leaking brown liquid which is accumulating in a bucket placed by establishment. Observed metal bottom of left most cabinet FOH containing soda boxes for soda machine to be collapsed by the weight and corrosion occurring on sides of metal leading to gaps between metal shelf and floor. Please repair and maintain in good repair.
