PRIORITY VIOLATION-3-501.16(A)(1), P: Time/Temperature Control for Safety Food, Hot Holding. Observed the following time/temperature control for safety items in hot holding unit: cooked rice, cooked beef, au jus, cooked beans, enchilada sauce and gravy with internal temperatures measured to be 108-119*F. Per person in charge items were reheated more than four hours prior. Items were voluntarily discarded, see embargo. Person in charge will put in work order for unit to be repaired. Do not use this unit to hot hold items, discussed using four hour time as a public health control policy for items until unit has been repaired to hold 135*F or above.

Observed wall behind front and back food prep areas in disrepair with part of wall panel peeled away revealing gap. Observed area surrounding mop sink in disrepair with part missing revealing gap. Please repair, all surfaces must be maintained in good repair smooth and easily cleanable.
