PIC stated they are in the process of having the unused items removed. 5TH CONSECUTIVE VIOLATION Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods Observed missing and broken floor tiles in kitchen and dining room areas. Advised person in charge (PIC) that flooring and surfaces shall be maintained in good repair to ensure they are smooth and easily cleanable. PIC stated they will have the tiles repaired.

Kitchen was not storing or preparing and time/temperature control for safety foods at time of inspection. Core-2-102.12(A), C: Certified Food Protection Manager (CFPM): Establishment does not have a current CFPM. Advised Person in Charge (PIC) that the PIC shall be the Certified Food Protection Manager, and per paragraph 2-101.11, and be present during all hours of operation. PIC stated they will obtain a Food Protection Manager certification as soon as possible. The Environmental Services Department will begin emailing permit card(s) and permit invoice(s) to permittees in the coming months.
