An Imminent Health Hazard is Present
Observed outside grease trap backing up outside. Water supply for the establishment was shut off to the restaurant by the City of Tempe to slow the back up from the grease trap. Permit will remain suspended until approval to open has been received from this department. Restaurant management and mall management were informed to start repairs to the grease trap immediately. City of Tempe Compliance Inspector was onsite at time of this inspection.

Due to the violations noted on this inspection report, County legal action may result from this inspection. Maricopa County policy requires suspension of the operating permit of businesses with serious conditions constituting an imminent public health threat. Your permit is hereby suspended. If desired, you may request a hearing within 15 days. When violations are corrected call this office for a reinspection to reinstate your operating permit. If violations are not corrected or if a hearing is not requested within 25 days, your operating permit will be revoked. Ride along with RS#789

e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced
food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance
with Cutting Edge program requirements receive an “A” grade.
