Per employee, all items are made at establishment were made yesterday. Employee date marked all items at time of inspection. Discussed date marking requirements with employee and person in charge (PIC). All RTE/TCS foods must be date marked after 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Discussed the importance of discarding all RTE/TCS foods after 7 days.

Core-3-602.11, C: Food Labels - Observed portioned sour cream and Oaxaca cheese in containers that are repackaged in the establishment with labels from the manufacturer labels on the containers. Observed Oaxaca cheese in containers with the manufacturer label for sour cream on it, with sour cream crossed out and Oaxaca written. Per employee, establishment buys bulk sour cream and Oaxaca cheese from the manufacturer and portions it into smaller containers at the establishment. Discussed with employee about proper labeling requirements and about putting their own labels for when items that are repackaged into containers onsite. Employee printed proper labels and labeled items at time of inspection.

Priority Foundation-6-501.111 (C), Pf: Controlling Pests; Approved Trapping Methods/Existence - Observed several live German cockroach near the floor drain located near the walk-in. Eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection on 5/18.

Priority Foundation-5-103.11, Pf: Capacity-Quantity and Availability - Observed hot water to be between 83 to 86*F at all sinks in the establishment. Owner stated water heater was recently replaced. All handwashing sinks must have hot and cold water, must be maintained in good repair, and reach a temperature of 100*F. All facility sinks such as mop sinks and 3-compartments sinks must have hot and cold water, and reach a temperature of 110*F. Reviewed hot water policy with owner. Plumber came out during inspection and restored hot water to all sinks in the facility.

Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods - Observed multiple holes in the walls and ceilings of throughout the establishment. Observed gap under the door to the exterior from the kitchen back hallway to the bathroom allowing visible light to pass through from the exterior. Outer openings must be protected from the exterior at all times to prevent the entrance of pests. Discussed the importance of maintaining walls smooth and easily cleanable. Re-seal so that areas are smooth, easily cleanable, and prevent the seepage of liquid. Repeat violation. Repair prior to next routine inspection.

This establishment is not participating in the award program at this time and had 0 Priority, 3 Priority Foundation and 3 Core Violations on this inspection. Core-2-102.12(A), C: Certified Food Protection Manager Observed no Certified Food Manager (CFM) onsite at time of inspection. Person in charge was unable to produce a certified food protection manager certificate. A CFM is required to be on the premises during all hours of operations. Provide a CFM onsite by next routine inspection. Repeat violation (3rd consecutive). Of Note: - Observed 30 gallon hot water heater at time of inspection. Plumber restored hot water in establishment at time of inspection. Discussed proper sizing guidelines necessary for hot water heaters with establishments that have 3-compartment sinks. Install approved 50 gallon hot water heater prior to 5/18. - The Environmental Services Department will begin emailing permit card(s) and permit invoice(s) to permittees in the coming months. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal.
