PRIORITY VIOLATION-5-205.12 (A), P: Prohibiting a Cross Connection Observed a hose connected from mop sink to a chemical dispensing unit below the atmospheric vacuum breaker (AVB). In order to protect the integrity of the AVB and to ensure proper backflow prevention the connection between the chemical dispenser and the mop sink should be disconnected when not in use. Manager removed the connecting hose. The cause of the violation was determined to be janitors using the mop sink previously and not disconnecting it after they were done using it. Manager spoke with the janitors to remind them to disconnect the connection when finished using it.
