The following email was sent to the operator. "If you plan to renew your permit, please contact us at 602-506-6824 (Monday-Friday 8 AM-5 PM) to schedule an inspection and/or pay for your operating permit. Please read the attached insert for payment options. If you have had an inspection within the last month, an additional inspection is not required to renew your permit. If your last inspection was not conducted within the last month, please contact us to schedule an inspection. We can schedule the inspection at a location that is convenient for you. Please ensure you provide the Department with an updated commissary agreement as required for the renewal of your operating permit. Once the permit fee is paid and the commissary agreement is received, we will mail out your updated sticker(s). If you do not plan to renew your permit, please reply to this email along with your name, business name, & permit #, confirming that you wish to close out the permit". No County legal action will result from this inspection.

Priority violation is a major violation that directly contributes to increasing the risk of foodborne illness or injury.
e.g. - Food employees do not properly wash hands when required
Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
