PRIORITY VIOLATION-5-202.11 (A), P: Approved System - Mop sink is not draining and has black water stuck in the basin. PIC stated an employee had poured oil down the drain which caused it to clog up and have a slow drain. The drain was slow until today when it appears to have been clogged. PIC stated a work order is in place to have the drain cleared. Mop water will be poured down floor sink at 3 compartment sink until the mop sink is repaired. Please have the mop sink repaired prior to re-inspection within 3 days. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained - AVB at mop sink is missing its protective cap. Please repair prior to next routine inspection. ***3rd consecutive violation***

Core-6-201.11, C: Floors, Walls and Ceilings-Cleanability - Coved base near on both sides of walk-in door entrance have gaps between them and the wall. The same type of gap is present on coved base on opposite wall of the office area. FRP wall panel near back door is flexing away from the wall and debris is accumulating behind. Corner panels that have recently been repaired near food prep sink are still missing coved base and the lower internal wall is exposed. Caulking at main hand wash station is beginning to crack away from the wall. Caulking at mop sink has small holes exposing the gap between the basin and wall. Ceiling panel above water heater has spaces around pipes exposing the internal ceiling. Please have these areas repaired prior to next routine inspection.

This establishment received a B Grade and had 1 Priority, 0 Priority Foundation violations on this inspection. Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days. Remodel includes salad (diced tomato, chopped lettuce, crumble scramble egg, etc) and hot bars (marinara sauce) with new floor drains below them, keg cooler, and small glass-front cooler (currently just used for glass cups).
