Inspection Comments

New business owner inspection. The establishment is not to the public. The permit is not approved at this time. Correct the following items for permit approval: (1) Provide FRP to the wall behind the 3 comp sink at the bar. FRP to extend 12 inches minimum above backsplash, and down to the coved base. (2) Provide FRP to the bar die wall, to extend the entire length, from under the bar top to the coved base (3) Label the second ice bin that has the glass rinser as "ice not for consumption" (4) Provide 1 inch air gaps above the floor sinks for bar drain lines. Provide dome strainers in all floor sinks where missing (5) Provide an RPZ backflow assembly (ASSE 1013) on the water line isolating the bar glass rinser. Provide funnel and drain assembly under the backflow, routed to the floor sink. Have backflow tested. (6) Provide a hub drain for the prep sink. (7) Replace the burnt out light bulb in the hood canopy (8) Remove the wood strip from the kitchen ceiling, patch and paint the areas of the ceiling where needed. (9) Provide split-front, commercial toilet seats in the employee and customer restrooms. (10) Provide self closing mechanisms on the employee and customer restroom doors. (11) Provide a self closing mechanism on the back door. (12) Install the window as planned in the dry storage room. (13) Caulk all holes in the walls, paint touch up where needed. Contact the Department to schedule a follow-up inspection at 602.527.0893 once all items have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
