Inspection Comments

New permit inspection walk through. The following items were identified for correction: 1). Install missing cleanout covers at prep sink and at the hand wash sink. 2). Install missing escutcheons at water and waste water lines in bathroom. 3). Repair broken tile where missing, use base tile with integral cove. 4), Where tile is loose, remount and grout or caulk to the wall. 5). The faucet at the food prep sink is leaking, repair - looks like washer at the faucet neck. 6). Repair the light switch for the light in the walk-in, if needed install a new switch if that panel is hard to replace. 7). Replace broken light bulbs at the light above the prep sink. 8). Re-install missing light shields or make sure that the lights are plastic. 9). Repair the two leaking valve stems at the three compartment sink. 10). Store is wet stacking dishes due to lack of space for air drying, please provide additional drying rack or change logistics. 11). The sanitizer concentration is set way to high and has to be diluted from the dispenser, adjust to proper concentration 150-400 PPM Quat 40 or 200 PPM for older Quaternary Ammonia products. 12). Under the soda station, re-test the Reduced pressure zone backflow valve and provide test report. 13). Please clean the floor sink under the prep sink and remove all organic matter. FYI- the soda station is leaking water running right over your hand when dispensing diet coke. This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. It is also the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Manager certified, staff has food handlers cards, see if you can provide proof on site for all employees. Re-inspection will be conducted No County legal action will result from this inspection. Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
