Inspection Comments

New permit inspection walk through. Restaurant is open and operating. The following items were identified for correction. 1). Kitchen hand sink cold water knob is not working, please repair. 2). Prep sink faucet is leaking at the neck, please replace. 3). The chemical system water connection is leaking at the three compartment sink and the connection is made at a point where it is hard to turn on the cold water knob, stop lean and raise connection. 4). Sinks stopper valves will need to be checked and moved forward so they are accessible. 5). The grease trap lid is rusty and has deteriorated to the point that it needs to be replaced. All caulk will need to be removed nd new lid caulked in. 6). Install missing clean out cover at the kitchen hand wash sink, one at the mop sink is damaged. 7). Install new split escutcheons at the drain lines in the mop sink cove and install where missing. Install elbow down on drain pan drain. 8). Clean and detail the soap and sanitizer holders under the three compartment sink. 9). Caulk the holes in the FRP at the side of the mop sink. 10). Where base tile stands open at the top wall juncture, please re-grout, also at tile in the front of the house at the rolling cookie case. 11). Please clean gap and re-silicone the hand sink cabinet with the cabinet with slicer. 12). Please provide a test report for the backflow valve under the soda station that is less than one year old or have the valve re-tested. Note that the chemical sanitizer dispenser is set very high at 400 PPM with tap water added. There are some rags on top of the freezer that were there for a past leak, front and back of the refrigeration unit. Please remove. This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. It is also the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Permit issued Eating and Drinking 10+ Class III No County legal action will result form this inspection. Re-inspection will be conducted 5/24/2023. Please make corrections before that date. Note that if additional follow up inspections are required you may be charged a inspection fee.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
