Observed bastorma (Arabic type sausage), in glass freezer case near front of meat department without any identifying origin labels. Person in charge stated they purchased the sausage from a distributor in San Diego and another distributor in Phoenix, however they were not able to locate a receipt or invoice demonstrating the sausage was produced from an approved source. Advised PIC that an invoice or receipt must be maintained to ensure all food items must are from approved, inspected sources. PIC had the sausage removed and stated they will obtained receipts/invoice for the sausage.

Observed 3-compartment sink without sanitizer. Upon questioning of two food service employees working in meat preparation area, neither knew how to prepare a sanitizer solution and verify concentration with sanitizer test strips. Advised food service employees and person in charge (PIC) that meat cutting equipment and cutting utensils must be washed, rinsed, and sanitized. Filled sanitizer compartment with water and chlorine. Demonstrated to food service employees and PIC how to prepare chlorine sanitizer solution in 3-compartment sink and verify chlorine sanitizer concentration using sanitizer test strips.

PRIORITY VIOLATION-7-206.11, P: Restricted Use Pesticides, Criteria Observed an aerosol can of "Raid" insecticide spray being store inside mop sink in meat processing area. Advised person in charge (PIC) that pesticides should not be stored on the premises. PIC removed pesticide can. Pest control activities (especially those including pesticide application) should be done by a certified pest control service/company/technician.

Core-MCEHC Chap. 8, Sec. 2, Reg. 5d, C: Latex Gloves Observed boxes of latex gloves under prep table in meat department. Advised person in charge that only vinyl, food grade type gloves are to be used when handling food. PIC removed the boxes of latex gloves and replaced with vinyl gloves. PIC stated they will ensure latex gloves are not used when handling food.

This establishment received a(n) D Grade and had 3 Priority, 0 Priority Foundation and 2 Core violations on this inspection. Core-2-102.12(A), C: Certified Food Protection Manager (CFPM): Establishment still does not obtained a CFPM certification. Advised person in charge (PIC) that a CFPM certificate must be obtained within as soon as possible or within 90 days of this inspection and maintained at the establishment. Provided link to County website for CFPM resources; www.esd.maricopa.gov. This is the 4th consecutive violation. Observed installed charcoal grill with hood being in back area behind meat department. Grill was not being used at time of inspection. However, observed boxes of onions, tomatoes, and bags of charcoal being stored near the grill. Person in Charge (PIC) stated the grill will eventually be used to serve customers, however only the employees are currently using the grill to prepare food for themselves. Advised PIC that the grill should not be used until it has been properly approved by Maricopa County and City of Phoenix. Establishment has a current Maricopa County Cease and Desist order in effect for the grill. Person in Charge must not operate grill for any reason until a remodel permit has been submitted and the grill has been approved to operate. Go to www.esd.maricopa.gov to submit for a remodel permit. Due to the violations and observations noted on this inspection report, County legal action may result from this inspection. Emailed inspection report.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
