PRIORITY VIOLATION-3-501.16(A)(1), P: Time/Temperature Control for Safety Food, Hot Holding: Cooked steak, and chicken in hot holding unit were at temperatures between 107 and 120° F. Products were reheated upon request. Cooked mixed vegetables were on stove at 84° F. Employee on duty stated that they just started and did not know how long they had been there. Vegetables were discarded.

PRIORITY VIOLATION-3-501.16(A)(2) and (B), P: Time/Temperature Control for Safety Food, Cold Holding: Cooked breaded chicken In pan on ice that was almost completely melted, was at 46-51° F. Once again employee did not know timeframe of the product. Chicken was discarded. Food products should not be held on ice unless the ice will be maintained and is at the level of the product.

PRIORITY VIOLATION-5-202.11 (A), P: Approved System: Mop faucet that was equipped with an atmospheric vacuum breaker had a y-valve attached and both water valves were left on. Water was turned off and y-valve removed. No valves including sprayers are permitted downstream from a non-testable backflow prevention device. Documentation was provided to permanently correct provided.
