At the hand sink in the room with the ice maker and the hand sink by the three compartment sink, the paper towel dispenser was out with no other hand drying options available. The hand sink in the front of house by the soda machines had paper towels in the machine but were inaccessible because the dispenser was locked and towels were not pulled through bottom of the dispenser. Every hand sink must have available hand drying provisions at all times. Manager replaced and made paper towels available at all three hand sinks.

PRIORITY VIOLATION-5-205.12 (A), P: Prohibiting a Cross Connection At the mop sink, observed a Y valve below the atmospheric vacuum breaker (AVB); one side connected to a chemical dispenser not in use and the other side was an open hose. Y valves are not approved to be used downstream of the AVB. Also, chemical dispensers should be disconnected from the mop sink when not in use. Both of these connections can compromise the integrity of the AVB. Manager disconnected the y valve and disconnected chemical dispenser.
