Observed employee handling raw frozen hamburger patties with gloved hands then start to prepare a sandwich without removing gloves and washing hands. Advised employee and person in charge (PIC) that gloves must be removed, and hands properly washed after handling raw animal foods. Employee discarded the sandwich buns, removed gloves, and properly washed hands.

Core-3-305.11, C: Food Storage-Preventing Contamination from the Premises Observed a bag of whole onions being stored on floor of walk-in refrigerator. Advised person in charge (PIC) that food shall be stored a minimum of 6 inches above floor to prevent contamination from premises. PIC moved the bag of onions to a storage shelf, 6 inches above floor.

Core-4-501.11, C: Good Repair and Proper Adjustment-Equipment Observed metal ice scoop at main ice machine, worn with pieces of metal missing from the scoop edge. Advised person in charge (PIC) that utensils shall be inspected, and maintained in good repair to prevent physical contamination of foods. PIC replaced the ice scoop with another scoop that was in better condition.
