OC Inspection. Walked with Craig and Ben. Correct the following items within 30 days: 1) Install a hand sink in dining area. Hand sink must have hot and cold water and drain to the sewer. Otherwise, cease open food service (ladling soup, scooping ice, portioning salad, etc.). 2) Install an RPZ (ASSE 1013) on water line for RO system, upstream of RO filters. Have RPZ tested and maintain certificate on site for inspection. 3) Install dual check valve backflow preventers (ASSE 1022) on water lines to coffee/tea breakers, steamer, steam table. 4) Prep sink cannot be directly connected to sewer – re-plumb prep sink so that it drains to floor sink under hood. Ensure drain line has a 1” air gap. 5) 3-compartment sink cannot be directly connected to sewer – re-plumb 3-compartment sink so it drains to floor sink at ice machine. Ensure drain line has a 1” air gap. 6) Install an atmospheric vacuum breaker on water line to garbage disposal; otherwise, remove garbage disposal. AVB must be installed a minimum of 6” higher than any downstream plumbing and be visible for inspection, and no shut-offs can be installed downstream of AVB. 7) Switch water lines at 3-compartment sink so that both faucets have hot on the right, cold on the left. 8) Have chemical company provide true minimum 1” air gaps inside chemical dispensers at 3-compartment sink (ASSE 1055A). ASSE 1055B is not a true air gap and is not approved. 9) Install a light at linen storage area. 10) Detail clean establishment top to bottom, notably under equipment and hand sink faucet. 11) Re-silicone hood and all sinks/stationary tables. Remove old silicone and clean surfaces first. Also seal gap at prep table across from cook line. 12) Remove damaged blinds from rear entrance door, as they are not easy to clean. 13) Ensure new refrigeration is installed with casters for ease of cleaning. Provide casters on existing refrigeration if not replaced. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
