Inspection Comments

Final construction inspection. Walk through with the superintendent and the chef. Construction is complete and approved. Superintendent will follow up on the items below before opening: 1). Drain lines in bar will be strapped down. 2) Soda gun holster line will be zip tied to the drain, keep air gap. 3). See of the foam in the two chases at the bar cab be smoothed out. 4). Provide a dual check valve for the tea brewer that meets plumbing standard ASSE 1022. 5). Repair leak on the three compartment sink. 6). Mount the water line under the dish washer off the floor. 7). Adjust the fan blade tp 3% outward. 8). Caulk or flash the hood at the back wall juncture, caulk to the side wall. 9). Fill in penetrations in the walk-in where refrigeration unit hangers were. Finish out smooth. 10). Caulk around the electric conduit in the walk-in freezer and seal. Make sure that the chemical dispenser can be installed on the backflow valve and not the mop ink faucet. Micro switch for the windows at the bar can be moved to the widows if desired so they do not come on with the lights. This restaurant will have a bar that is open to the exterior when windows are opened. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so.
