Final construction inspection conducted with superintendent Jim by appointment. Inspection rights were delivered via email. The establishment was closed and not operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department?s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the construction inspection within 10 days: 1) Adjust the drain lines at the prep sink and bar sinks to ensure 1" minimum air gaps are provided and heavy splashing on the floor is reduced. 2) Provide a splash guard to the right handwash sink located at the cook line. 3) Provide additional clear epoxy coats to the exposed block walls in the restrooms to prevent the mortar from falling off and to fill the wall holes and gaps. 4) Caulk-in the escutcheon plates and plumbing penetrations throughout the facility with silicone. 5) Caulk the ventilation hood gaps with an aluminum silicone. 6) Replace or resurface the rusted shelving unit located next to the warewash machine. 7) Provide lighting to the walk-in cooler located near the prep sink. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
