OC Inspection for class 4 E&D 10 + seating Establishment is open and operating Permit is approved Inspection was conducted with F&B director Tim R. and RS #1028 The following items must be completed prior to OC reinspection by 5/14. 1) Provide hot water at the hand sinks next to the the 3 compartment sink, and prep sink. Hot water must be between 100-110'F 2) Provide ASSE 1022 backflow devices for the coffee machine, espresso machine and ice tea machine. 3) Certify all ASSE 1013 backflow devices. 4) Repair drain pipe to the hand sink near the prep sink to prevent any water leaking, and ensure all hand sinks drain properly. 5) Provide/install hard drains to the water softener near the dish machine, and provide a 1" air gap to the floor sink. 6) Smoke residue observed on ceiling. Clean smoke residue from the ceiling, and paint if needed. Per PIC, vents were just deep cleaned and PIC will monitor the ventilation for the fire place if any further corrective action is needed. 7) Remove plastic on the front of the main hood vent. 8) Replace any missing tile observed at the front of the pizza fire brick oven. 9) Removed any unused equipment stored outside of the main back door kitchen. 10) Seal the back of the hand sink to the wall next to the fryers. 11) Provide a non-consumption sticker/document for the ice on the ice coolers attached to the golf carts. 12) Observed a hand wash sign located on the splash guard of the dump sink. Remove sign and add a hand wash sign to the hand wash sink. 13) Clean inside of the bottle coolers located in the bar area. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
