OC re-inspection - E&D Class 4 This inspection was conducted with the facility managers A PDF copy of this inspection report was emailed to Dan L. at the time of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the following prior to re-inspection scheduled for 10/28/2021; 1. There is currently copper and brass on the downstream side of the RPZ backflow prevention device for the RO system to the new Combi-oven. All piping and fittings on the downstream side of this backflow device must be plastic or stainless steel, no copper or brass. 2. Replace the broken atmospheric vacuum breaker for the submerged water inlet in the scupper drain adjacent to the main mechanical dishwasher and three compartment sink. 3. Replace the chemical dispenser in the mop sink area of the bar with a chemical dispenser that has a proper air-gap. 4. Provide a minimum of a semi-gloss paint finish to the bare wood on the underside of the bar top. Also, caulk and seal the gap between the underside of the bar top and the supporting wall beneath. 5. Repair the damaged cove base tile at the floor/wall juncture inside the kitchen walk-in refrigerator/freezer unit. No county legal action will result from this inspection.
