E&D, class 3 10+ Final construction inspection was conducted with the person in charge, Paul. Construction has been completed in accordance to plans. The establishment is not open and operating. Per Paul, mister system, misting fan or swamp cooler will not be installed on the future patio. Per Paul, the establishment would like to add a dog friendly patio when they build the patio. Please submit application for dog friendly patio permit prior to constructing the space. Permit is approved. The manager is a certified food protection manager. Please ensure that there is a certified food protection manager on site during operational hours. Please address the following items prior to the routine inspection: 1. Provide an ASSE 1022 standard, vented dual check valve, backflow preventer at the water connection point of the coffee/ tea brewer. This is not a testable device so replace it annually or whenever it leaks; whichever comes first. 2. Provide dunnage rack or storage racks for the food and kegs inside the walk-in unit. 3. Provide missing plugs inside the walk-in unit. 4. Seal the gap at pipe/ conduits penetrations and between panels inside the walk-in unit. 5. Caulk and seal all gaps, cracks and penetrations throughout. This includes inside the walk-in unit and under the hood as needed. 6. Provide closet with door(s) or curtain from ceiling to floor for the washer/ dryer. 7. Certify the testable backflow preventer and provide report. The inspection rights sheet will be emailed with the report to Paul. No County legal action will result from this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.


Definitions
Priority violation is a major violation that directly contributes to increasing the risk of foodborne illness or injury.
e.g. - Food employees do not properly wash hands when required
Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
