Inspection Comments

OC inspection conducted with Kirk H. by appointment. Inspection rights were delivered via email. The establishment was closed, but is currently operating. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Install a 50 gallon capacity minimum water heater. 2) Provide a dedicated prep sink to the bar that is indirectly plumbed to the floor sink or provide a sign letter indicating that prepped garnishes will be prepared at the bar next door (under same ownership) and brought to the facility. 3) Trim or bracket the drain lines for the ice machine and three compartment sink to provide 1" minimum air gaps above the floor sinks. 4) Provide ASSE 1055A internal air gaps to the chemical dispensers located at the bar and mop sink room. 5) Remove the flex hose on the drink rail and plumb the drain with rigid piping to the floor sink with a 1" minimum air gap. 6) Provide rubber cove base to the floor/wall junctures of the upstairs dry storage rooms and the mop sink room. Seal the base of the rubber base with silicone. 7) Paint the exposed wall areas in the dry storage rooms with semi-gloss paint. 8) Fill all wall holes throughout the dry storage rooms and at the bar, as needed. 9) Replace the rusted and corroded metal cove base in the walk-in cooler. 10) Seal the penetrations in the walk-in cooler with silicone. 11) Refinish the concrete flooring in the ice machine room to provide a smooth, durable, non-absorbent, and easily cleanable surface. 12) Provide a finger or tool cove to the floor/wall juncture of the ice machine room. 13) Provide partitions between the men's urinals. 14) Provide air curtains above the front windows with micro-switches or provide an approved integrated pest management program. Information was provided. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
