Inspection Comments

OC inspection conducted with employees by appointment. Inspection rights were delivered via email. The establishment was open and operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Provide a 50 gallon capacity water heater. Observed a 40 gallon installed. 2) Provide ASSE 1013 reduced pressure zone backflow preventers upstream of the pitcher rinser and glass rinsers. Multiple rinser units can be installed downstream of one backflow preventer, as long as the water supply dead legs. Certify and provide test reports. 3) Trim or lift and bracket drain lines going to floor sinks to ensure 1" minimum air gaps are provided. 4) Remove the flex drains for any glass rinsers and install rigid piping. Ensure 1" minimum air gaps are provided. 5) Provide a hose bib vacuum breaker for the hose bib located at the exterior of the building. 6) Provide ASSE 1022 dual check backflow preventers on the water supply lines connected to all non-carbonated beverage equipment. 7) Disable the manual switches on the drive-thru air curtain units. The units were turned off at time of inspection. 8) Caulk-in escutcheon plates and plumbing penetrations with silicone. 9) Reseal sink fixtures throughout the facility, as needed, with silicone to the walls. 10) Provide cove tile to the floor/wall juncture near the safe. 11) Provide a u-shaped toilet seat in the restroom. 12) Repair the ventilation fan in the restroom. 13) Replace the chipped and rusted floor sink grates throughout the facility. 14) Flash off the top of the walk-in cooler and remove the items being stored on top. Reviewed sanitizer solutions at time of inspection. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
