Inspection Comments

OC inspection conducted with multiple management members by appointment. Inspection rights were delivered via email. The establishment was open and operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Install an ASSE 1013 reduced pressure zone (RPZ) backflow preventer upstream of the soda carbonator. Ensure no copper is located downstream. Certify and provide a test report. 2) Indirectly plumb the three compartment sink to a floor sink with a 1" minimum air gap. Observed the sink directly plumbed. 3) Provide a longer faucet for the three compartment sink to ensure all compartments can be properly filled. 4) Repair or replace the insta-hot unit in the mens restroom to ensure hot water measuring 100' F minimum is provided. 5) Transition the flex drain at the soda fountain to rigid piping and air gap the pipe to the floor sink (1" minimum). 6) Caulk-in all escutcheons and plumbing penetrations throughout the facility, as needed, with silicone. 7) Re-caulk all sink fixtures throughout the facility, as needed, with silicone to the walls. 8) Fill all wall holes, as needed, throughout the facility. 9) Replace the brick supports for the soda carbonator with a plastic dunnage like rack. 10) Lift the shelving in the small storage room to ensure 6" minimum clearance is provided. 11) Relocate the prep table near the main handwash sink to provide a direct 25 feet walk to the handwash sink from the far south prep table. 12) Replace the torn refrigerator door gaskets throughout the kitchen, as needed. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
