OC Inspection, E&D Class 3, 10+ Seating. Inspection conducted with owner Tucker B. Permit approved This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Correct the following items prior to reinspection on 8/2/21: 1) Provide a dedicated water line for the chemicals at the mop sink or remove the chemical dispenser from the mop sink wall. Observed a Y-valve connected downstream of the mop sink faucet (equipped with an AVB). PIC removed the Y-valve and connection to the chemicals at the time of inspection. 2) Test and certify the RPZ backflow preventer behind the bag in box (ASSE 1013). Provide test report. 3) Install a splash guard on both sides of the 3-compartment sink (stainless steel, plexi glass, or tempered glass). Observed a shower curtain on both sides of the 3-compartment sink. All surfaces must be smooth and easily cleanable. 4) Repair drive thru window mechanism so that the doors completely self-close. 5) Reseal the back of the 3-compartment sink using a bead of silicone. 6) Reattach any lose or missing escutcheon plates throughout kitchen. Noted lose or missing escutcheon plates on the wall next to the prep sink, pipes near the wall of the mop sink, left side handle of mop sink faucet, and the hole above the bag in box. 7) Deep clean walls, ceiling, ceiling vents in kitchen, ceiling above the make tables, exhaust fans in restrooms, and in between equipment. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD Inspection report emailed to owner Inspection rights emailed to owner. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
