Observed employee take out trash and washed hands for five seconds before going to cut artichokes. Had employee re-wash hands for 20 seconds. Hands must be scrubbed for 20 seconds, rinsed with warm water, and dried with air dryer or new towel to prevent contamination.

PRIORITY VIOLATION-5-202.11 (A), P: Approved System Observed a Y-valve connected downstream of the mop sink faucet (equipped with an AVB). One side was connected to a hose with spray nozzle head and the other line was attached to chemicals. Manager removed the Y-valve and connection to the chemicals. Provide a dedicated water line for the chemicals at the mop sink or keep detached while not in use. Spray nozzle heads must be removed promptly after each use.
