OC Inspection, E&D Class 3, 10+ Seating. Inspection conducted with manager Rosa and supervisor Lym C. Inspection report and inspection rights emailed to Lym C. ***Establishment receives all their produce pre-washed and pre-cut. Establishment can not wash or cut any produce without a prep sink. If the menu or the process to the menu items change consult with the department. Permit approved This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Correct/address the following items prior to reinspection in 2 weeks (November 24th): 1) Provide an ASSE 1013 RPZ backflow device for the blender rinser. Ensure that the RPZ has a funnel/hub drain underneath the relief port that drains into a floor sink with a 1-inch air gap. 2) Provide ASSE 1022 dual check valves for all non-carbonated beverage equipment including the steamer. Ensure the ASSE 1022 are either stainless steel or plastic (no brass). 3) Provide an approved NSF/ANSI hand sink (12x12x6) on the wall next to the ice machine. Provide hot and cold water and have it plumb directly. Provide splash guards on both sides of the hand sink. Relocate the iced tea dispenser as discussed. 4) Replace the AVB backflow device at the mop sink. Water was observed leaking from this backflow. 5) Test and certify all RPZ backflow devices and provide the test reports. 6) Lift all drain lines up out of the floor sinks and provide a 1-inch air gap between the drainpipes and the floor sink. 7) Transition all flex tubing that is draining into a floor sink with a rigid drain line. 8) Lift all plumbing lines up off the floor. 9) Repair/replace the reach in cooler neat the drive thru window. Per manager they have a work order in for this unit. Ensure the ambient air temperature is at 41’F. No food was stored in this unit at the time of inspection. 10) Replace burnt out lights within the walk-in freezer. 11) Remove the Flotec pump. This pump is not connected to anything. 12) Detail clean underneath equipment and under the soda equipment in the lobby area. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. ***Additional fees will be assessed for any additional follow-up plan review inspections. No County legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
