Permit re-inspection with the owner walk. Some items were corrected. Please continue with the left over items below: 1). Install small piece of base at the entrance of the men's room. 2). Replace damaged and missing ceiling panels in the side room near walk-in entrance. 3). Repair wall and replace base in the room with entrance walk-in. 4). Clean the floor under the bag in the box rack. 5). Remove old caulk and re-caulk the hand sink in the men's room. 6). Repair tile in the men's room or grout in. Cover with mirror. 7). Replace urinal partition in the men's room with one that meets code 12 inches from floor max, 60 inches min height, extending 6 inches in front of lip urinal. Or install lock on the door. 8). In support room, provide cleanout covers. 9). Support room, fill in all holes in the walls. 10). Provide a washable ceiling over the sinks and mop sink. 11). Repair wall where open and replace - remount coved base. 12). Air vent in the dish room is depositing rust and debris, investigate clean and replace if rusted. 13). There are areas with broken and damaged VCT tiles. repair where in dis-repair. 14). Or disable the exterior hose bib at the old restrooms or install a hose bib vacuum breaker. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Permit approval does not mean that items on the list do not need to be corrected. Please make corrections and call for your re-inspection within 30 days. Please note that you will be required to pay a $240.00 fee for the next inspection. Permit issued Retail Grocery Class II No County legal action will result from this inspection. Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
