Inspection Comments

OC inspection conducted with managers Jon and James by appointment. Inspection rights were delivered via email. The establishment was closed, but is operational. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Provide an additional handwash sink at the soda fountain area that will service both the soda fountain and ice machine. Indirectly plumb the sink to a floor sink with a 1" minimum air gap. 2) Install ASSE 1013 reduced pressure backflow preventers upstream of the bar glass rinsers and misting system. A single backflow device can supply multiple backflow units as longs the water line dead legs after the rinsers. Certify and provide test reports. 3) Reconnect the soda carbonator to the RP backflow preventer located under the soda fountain. Provide a test report. 4) Provide an ASSE 1055A internal air gap to the mop sink chemical dispenser. 5) Lift and bracket the bar indirect drain lines to provide 1" minimum air gaps above the floor sink. 6) Disable the manual switches for the air curtains located throughout the facility. The air curtains must only be operated by micro-switches solely. 7) Fill all wall holes throughout the facility, as needed, with silicone. 8) Caulk-in al escutcheon plates and plumbing penetrations throughout the facility with silicone. 9) Caulk the fixed shelving units to the wall with silicone. 10) Relocate personal items from the small dry storage room to prevent contamination. 11) Repair the pitted flooring at the scullery area to provide a smooth, durable, non-absorbent, and easily cleanable surface. 12) Reseal sink fixtures throughout the kitchen with silicone. 13) Refinish or replace the rusted shelving units in the walk-in cooler. 14) Clean floor sinks and replace any rusted floor sink grates. 15) Provide a splash guard to the north bar handwash sink on the right side. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. Reviewed sanitizer solutions, date marking, and raw protein storage. Reviewed dog friendly patio requirements. Facility will be submitting for a dog friendly patio prior to October 31st, 2021. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
