Inspection Comments

E&D, class 4 10+ OC inspection was conducted with the person in charge, Tiffany. The establishment is open and operating. Per owner, the establishment may remodel the restrooms in the future. Please submit application for this scope of the work at least 30 days before demolition commence. Permit is not approved. The inspection rights sheet will be emailed with the report to the person in charge. Please address the following items prior to the re-inspection: 1. Provide a 50-gallon tank water heater. The current 40-gallon tank water heater is not acceptable. 2. Provide a hand washing sink in the cooking/ food service area. Sink must have hot and cold water and can be directly connected to the sewer. 3. Provide a mop sink faucet for the mop sink area. Faucet must have hot and cold water. Currently, the establishment only have one spigot of hot water. 4. Provide 1” air gap from the finished floor for drainpipes from ice machine, ice bin, prep sink, drip tray of soda fountain drink station. 5. Provide an ASSE 1022, vented dual check valve backflow preventer, at the water connection point of the soda carbonator and another one for the tea brewer. These are not testable backflow device so provide a written policy for maintenance and monitor for leakage and annual replacement. 6. Provide several coats of polyurethane on the laminate countertop of the restroom’s lavatory and soda fountain station. Laminate countertop is not acceptable in wet areas. 7. Noted a fan with blade is used at the delivery door for air curtain purpose. Air curtain must be bladeless and activate by a micro-switch. Please replace the fan with an approved air curtain. 8. The door of the walk-in cooler is damaged and rusted. Please replace / refinish or provide diamond plate metal panel for the interior of the door. Ensure the edges are smooth and rounded if metal panel is used. 9. Caulk and seal gaps, cracks and penetrations throughout. This includes inside the walk-in cooler, behind sinks and under the hood. 10. Replace gaskets on refrigerator/ freezers as needed. 11. Please replace the household (Frigidaire) reach-in freezer as needed. Household equipment is not acceptable. No County legal action will result from this inspection. Please contact the Department to schedule a follow-up inspection at 602.506.6824 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. ***Additional fees will be assessed for any additional follow-up plan review inspections. You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code.” County legal action may result due to violations observed during this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
