Correct the items listed within 90 days or sooner provided supply chain issues are resolved. - Install a reduced pressure principle backflow prevention device (ASSE 1013) upstream of the produce display mister system. If the spray hose is connected to the same water line at the mister system, and the water line dead legs, then the one device will be sufficient. If the sprayer is on a separate water line, or the water line does not dead leg at the produce area, then an additional device (ASSE 1013) will be required for the spray hose. - Confirm the water connection at the free-standing produce tables are not connected to a water supply line. If they are, then a proper backflow prevention device (ASSE 1013) will need to be installed at each table. - Provide a stainless-steel splash guard on the left side of the hand washing sink at the produce area. - Remove the unused/non-functioning garbage grinder at the produce wash sink. They existing 2 basin sink can be made a dedicated vegetable prep sink for fruit cutting. - Install a dedicated 3 compartment sink for warewashing in the produce prep area. Ensure the sink has hot water, cold water, and a rigid drain line the empties into a floor sink with a 1 inch air gap.

Inspection Comments

This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are non-refundable. IF VIOLATIONS ARE NOT CORRECTED BY ESTABLISHED TIMEFRAMES, LEGAL ACTION MAY RESULT. New owner inspection conducted with Manager Brad S. Brad S. verbally acknowledged the inspection rights. Report emailed to Cristy Z. and store email. Please see the guide sheets for items to correct. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. Please contact the Department to schedule a follow-up inspection at (602) 506 - 6824 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
