Inspection Comments

New owner inspection walk through for the Deli permit with the store director. The following items were identified for correction: 1). Please install a reduced pressure principle upstream of the Rational oven. The water can be split after the valve so one valve should suffice. Install with hub drain assembly and have the valve tested and certified after installation. 2). After the next cleaning re-caulk the hood at the back wall junctures, please remove the old caulk. 3). Re-caulk the pizza hand sink at the top wall juncture. 4). Install coved base in the cove with the pizza prep table and on the small wall along the side where the chicken case used to be. 5). Provide a full drain grate for the floor sink where the chicken case used to be. 6). Re-caulk the hand sink on the back wall of the deli. 7). Provide missing light shields for the lights in the large deli case or replace with plastic LED. 8). Re-caulk the prep sink at the top wall juncture. 9). Raise the spray arm assembly to above the flood rim at the three compartment sink. 10). Remove old molded caulk and re-caulk the three compartment sink. 11). Provide a pressure vacuum breaker for the chemical dispenser at the mop sink and install 12 inches above the highest point downstream. Certify valve after installation. 12). The atmospheric vacuum breaker on the mop sink faucet is leaking, please repair. 13). The floor sink next to the water heater in the riser room is clogged, please repair. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Permit issued Eating and Drinking Class IV 0-9 Seating. This location does not conduct outdoor cooking. Correct the items listed within 90 days or sooner provided supply chain issues are resolved. Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
