Specific items for correction: 1) Replace the corroded and leaking pressure vacuum breaker installed upstream of the chemical spray system. Install the new unit at least 12" inches above the chemical spray system. 2) Provide splash guards to the left side of the seafood handwash sink and to the right side of the meat room handwash sink. 3) Repair the damaged ceiling areas, as needed, and repaint with semi-gloss paint. 4) Provide rubber cove base to the floor/wall junctures, where missing. Seal the base of the rubber base with silicone. 5) Repair the damaged wall areas around the entryway between the seafood and meat areas. 6) Provide a new Hobart meat testing box. The current unit is heavily rusted. 7) Lift the water lines under the three sink to provide a 6" minimum clearance from the floor. 8) Refinish the rusted ceiling areas in the walk-in cooler.
Inspection Comments

OC inspection for Class 4 Meat Marke permit conducted with management by appointment. Inspection rights were delivered via email. The establishment was open and operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please correct the items listed within 90 days or sooner provided supply chain issues are resolved. General items for correction: 1) Provide 1" minimum air gaps above the floor sinks for all indirect drain lines. 2) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility with silicone. 3) Fill all wall holes, gaps, and cracks throughout the facility with silicone. 4) Replace the rusted and chipped floor sink grates and provide the missing dome grates within the floor sinks. 5) Trim all foam sealant at penetrations in the walk-in coolers flush and provide a smooth layer of silicone. 6) Re-caulk department sink fixtures. 7) Refinish the chipped, corroded, and pitted flooring in the walk-in coolers and epoxy flooring throughout the entire facility, as needed, to provide a smooth, durable, non-absorbent, and easily cleanable surface. 8) Re-wrap the condenser and condensate lines in the walk-in cooler with new black foam where damaged foam is present. 9) Conduct a cleaning of all floors, walls, and ceilings, as needed. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
