Specific items for correction: 1) Install an ASSE 1013 reduced pressure backflow preventer upstream of the produce misting system and sprayer quick connect system. Certify and provide a test report. 2) Provide hot water measuring 110' F minimum to the prep sink faucets. Increase the pressure at the sink fixtures as well. 3) Provide an additional portable handwash sink to provide a backup in the event the main unit breaks down during operation. 4) Provide overhead protection, barriers, and fill all concrete cracks at the chile roasting area or relocate the operation to the front cooking area where chicken is done. 5) Remove or disable the manual switches for the receiving door and front entry air curtains. Install micro-switches for all air curtains. 6) Replace the double fan units at the main receiving door with an approved air curtain device. 7) Replace the damaged ceiling tiles in the back stock room area and replace the acoustical ceiling tiles above the mop sink with smooth, washable vinyl ceiling tiles. 8) Provide new weather stripping and door sweeps to the back exterior doors to prevent easy pest entry. 9) Provide rubber cove base to the floor/wall junctures of the cmu block walls throughout the entire back stock rooms. 10) Reattach and replace the scrim sheeting located above the dairy cooler area. 11) Repaint the ceiling above the produce prep sink with semi-gloss paint. 12) Provide a splash guard to the right side of the produce handwash sink. 13) Provide additional lighting in the product prep room to ensure 50 foot candles minimum is provided. 14) Refinish the walk-in freezer concrete cove, as needed, to provide a smooth and easily cleanable surface. 15) Repair the walk-in freezer ceiling areas that are damaged. 16) Repair the ventilation units in the restrooms. 17) Regrout the tile flooring in the restrooms, as needed, to provide a smooth and easily cleanable surface. 18) Provide small covered trashes women's stalls.
Inspection Comments

OC inspection for Class 3 Retail Food Establishment permit conducted with management by appointment. Inspection rights were delivered via email. The establishment was open and operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please correct the items listed within 90 days or sooner provided supply chain issues are resolved. General items for correction: 1) Provide 1" minimum air gaps above the floor sinks for all indirect drain lines. 2) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility with silicone. 3) Fill all wall holes, gaps, and cracks throughout the facility with silicone. 4) Replace the rusted and chipped floor sink grates and provide the missing dome grates within the floor sinks. 5) Trim all foam sealant at penetrations in the walk-in coolers flush and provide a smooth layer of silicone. 6) Re-caulk department sink fixtures. 7) Refinish the chipped, corroded, and pitted flooring in the walk-in coolers and epoxy flooring throughout the entire facility, as needed, to provide a smooth, durable, non-absorbent, and easily cleanable surface. 8) Re-wrap the condenser and condensate lines in the walk-in cooler with new black foam where damaged foam is present. 9) Conduct a cleaning of all floors, walls, and ceilings, as needed. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
