OC Inspection - Retail Food This inspection was conducted with the manager on duty. A PDF copy of this inspection report was emailed to Christy Z. on the day of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the items listed within 90 days or sooner provided supply chain issues are resolved; 1. Provide a certified, drinking water safe, RPZ backflow prevention device for the water supply line to the RO system for the produce case mister system. 2. The RO system for the produce case mister system is leaking onto a rag that has been set beneath it. Repair all water leaks in the RO system 3. Clean the trash and debris from the top of the produce cases. 4. Fill and seal the large seam in the floor of the Dairy Cooler. This seam extends into the back room and it must be filled and sealed in the back room area also. 5. The Produce max chemical dispenser at the two compartment produce sink does not have a proper air-gap. Replace this chemical dispenser with one that does have a proper air-gap. 6. There is a buildup of ice condensate around the coolant lines in the ceiling of the Produce Walk-in. Remove this ice condensate and remove the electrical tape from the ceiling around the coolant lines. No county legal action will result from this inspection.
