OC Inspection - E&D Class 4 - Deli This inspection was conducted with the manager on duty. A PDF copy of this inspection report was emailed to Christy Z. on the day of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the items listed within 90 days or sooner provided supply chain issues are resolved; 1. The exhaust hood over the rotisserie chicken oven is not functional. Repair this exhaust hood to be fully functional. 2. Provide a certified, drinking water safe, RPZ backflow prevention device for the water supply line to the soda fountain carbonator. All piping and fitting on the downstream side of this device must be plastic or stainless steel, no copper or brass. 3. Provide stainless steel on the walls behind the cooking equipment under the main exhaust hood in the deli. The metal must extend down to the cove base where there is currently only FRP installed. 4. Replace the damaged and missing metal cove base at the floor/wall junctures inside the deli, walk-in refrigerator. Also, repair the severely damaged floor area under the doorway to the walk-in freezer inside this walk-in refrigerator. 5. Replace the broken door seal on the deli walk-in refrigerator door.. 6. Repair the damaged floor underneath the deli ice machine to be smooth, non-absorbent and easily cleanable. 7. Repair the damaged base and seal the spaces around the plumbing pipes in the wall underneath the left end of the three compartment sink. No county legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
