OC Inspection - Bakery This inspection was conducted with the manager on duty. A PDF copy of this inspection report was emailed to Christy Z. on the day of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the items listed within 90 days or sooner provided supply chain issues are resolved. 1. Provide an NSF approved food prep sink in the bakery area. This sink must have separate hot and cold water valves and an indirect drain pipe that empties into a floor sink with a minimum 1 in. air-gap. 2. The RPZ backflow prevention device for the soda fountain in the bakery must be tested and certified. 3. The cabinet beneath the tea dispensers where the water filters are located is severely damaged and the water filters are resting loosely on the floor beneath the broken shelf. Repair the inside of this cabinet and properly mount the water filters onto the inside of the cabinet and not on the floor. 4. Provide a certified, RPZ backflow prevention device for the water supply line to the outdoor fountain filter system. There is currently a double check valve assembly installed that must be replaced with an RPZ backflow device. The filter system is located in a closet in the back bakery area. 5. There is a plastic, PVC pipe extending below one of the ice machines in the back bakery area. The purpose of this pipe could not be determined. Either remove this pipe or extend it so that it can drain properly into the floor sink with a minimum 1 in. air-gap directly beneath these machines. No county legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
