OC Inspection - E&D Class 4 - Deli/Bistro This inspection was conducted with the manager on duty. A PDF copy of this inspection report was emailed to Christy Z. on the day of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the items listed within 90 days or sooner provided supply chain issues are resolved; 1. There is a floor sink on the west end of the deli, nearest to the sushi area, that is partially obstructed by a cabinet shelf. This shelf is also in disrepair. Repair the damaged shelf so that it is easily cleanable and so that the floor sink is completely accessible. 2. The RPZ backflow prevention device for the water line to the Rational oven in the Deli must be tested and certified. This backflow device currently feeds the Rational oven and the nearby Evolution Steamer. It must be reconnected so that it only feeds the Rational Oven. This oven must be isolated from all other plumbing fixtures. 3. A separate, new certified RPZ backflow prevention device must be installed for the Evolution Steamer. The steamer must be isolated from all other plumbing fixtures. 4. The hot and cold water valves on the left faucet of the deli three compartment sink area reversed. Provide hot water on the left and cold water on the right for this faucet. 5. The RPZ backflow prevention device for the deli soda fountain must be tested and certified. 6. All chemical dispensers at the three compartment sink and food prep sink that do not have air-gaps must be replaced with chemical dispensers that do have proper air-gaps. 7. Clean the grease, soil and debris from the countertops and food equipment in the outdoor cooking area. No county legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
