E&D, class 3 10+ Equipment inspection was conducted with the person in charge, Hao N. and contractor, Tony. Construction had been completed. The establishment is open and operating. Informed owner that a permit is required prior to operating and an application to obtain the permit is not a permit to operate and instructed the owner to close the shop. Permit is not approved. Please address the following items in order to be approved: 1. Provide a hand sink in the ice machine room. 2. Provide another hand sink at the boba topping and ice cream dispensing case or relocate the equipment to the prep area. 3. Provide a backwater valve and a riser between the drainpipe from the 3-comp sink to the above ground grease interceptor. The top of the riser must be 1” below the basin of the 3-comp sinks. 4. Provide 1” air gap from the finished floor for the drainpipe from the ice bin and ice machine. Brace the pipe along the wall with metal brackets, slope to drain and indirectly discharge to the floor sink with minimum 1” air gap from the finished floor. 5. Provide a micro-switch activate air curtain at the drive-through door. 6. Provide solid countertop for the countertop of the drink/ food service areas (i.e. Corian, quartz, granite, stainless-steel. 7. Provide FRP panels, stainless steel or tiles behind the 3-comp sinks and mop sink. 8. Provide tiles on the floor at the drive-through door and behind the ice bin. 9. Provide sanitary toilet seat for the women’s restroom. 10. Provide self-closer or self-closing hinges on the restrooms’ door. 11. Provide washable ceiling tiles from the front edge of the counter to the back storage, mop sink, ice machine, water heater areas and restrooms. 12. Provide several coats of semi-gloss paint on walls in the food service, storage, ice machine, mop sink, water heater areas. 13. Provide 3/8” radius cove base in the dry storage, mop sink, and ice machine rooms. The inspection rights sheet is emailed with the reports to the person in charge. Please contact the Department at 602-506-6824 to schedule the next inspection. Allow 7-15 business days to schedule. You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code.” County legal action may result due to violations observed during this inspection. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
