Inspection Comments

OC inspection conducted with manager Joseph M. Manger acknowledged receiving information on inspection rights. Permit approved. Inspection report emailed to Beth M. and Manager. Correct the items listed within 90 days or sooner provided supply chain issues are resolved: 1. Provide a second tankless water heater to accommodate for the hot water use of the establishment. 2. Provide a rigid drain line for the make table on the prep line. Ensure to brace it so that it maintains a 1-inch air gap with the floor sink (currently is using a water hose). 3. Repair the leak from the from the 4-compartment sink faucet and prep sink faucet (leaks when in use). 4. Brace the drain for the self-serve soda dispenser so that it maintains 1-inch air gap with the floor sink. 5. Certify the backflow preventers upstream of both soda carbonators for the soda dispensers and provide a copy of the backflow preventer test certification. 6. Provide an ASSE 1022 backflow preventer upstream of the tea brewer. 7. Remove the silicone from the side of the 3-comp sink and backsplash, and reseal (black organic material has built up under the seal). 8. Repair the tile cove base adjacent to the rear exit, and adjacent to the walk-in cooler, and corner inside men’s restroom. 9. Replace the burnt light bulb in the walk-in cooler. 10. Clean underneath the self-serve soda cabinet (organic material spilled inside cabinet). 11. Seal up the side of the metal flashing inside the self-serve soda cabinet. 12. Seal the soda syrup line penetration behind the ice cream machine. 13. Seal the backsplash of the hand sink inside women’s restroom. 14. Seal the T&P relief drain of the tankless water heater that extends to the exterior (outside seal is worn). 15. Provide vinyl ACT ceiling tile for area above the carryout display monitor. 16. Provide cleanout covers where missing (under hand sink adjacent to ice machine and mop sink). 17. Remove the spray nozzle from the mop sink hose (spray nozzle was too tight to remove by hand, nozzle was placed above faucet until it can be removed). Of Note: Establishment was unable sustain hot water with a single tankless water heater. Hot water to the hand sinks while the 4-compartment sink was being filled lost pressure. Hot water measured 120°F at the 4-compartment sink, while the hand sinks at drive through and prep line measured between 77°F to 80°F during the fill process. The manager was brought over to verify hot water readings during the process. Please be advised that the New Business Permit Application fee includes one initial inspection and on follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
