OC inspection for a Class 3 10+ E&D was conducted with Ron T. by appointment. Inspection rights were delivered via email. The establishment was closed and not operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Provide hot water measuring 100' F to the front area handwash sink. 2) Indirectly plumb the three compartment sink to the floor sink with a 1" minimum air gap. Cap the direct drain line. 3) Trim the drain lines at the three sink floor sink to provide 1" minimum air gaps. 4) Fill all wall holes throughout the facility with silicone. 5) Caulk-in all plumbing penetrations and escutcheon plates with silicone. 6) Replace or refinish the rusted shelving units in the reach-in coolers, as needed. 7) Replace the torn refrigerator door gaskets, as needed. 8) Provide the missing side sneeze guard piece at the steam table located near the front handwash sink. 9) Repair the large crack in the steam table countertop located near the front handwash sink. 10) Replace the corroded faucets at the three compartment sink and back handwash sink. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
