E&D, class 3 10+ The establishment is approved for 30-transitional period. OC inspection was conducted with the manager, Charlie R. and facility engineer, John E. The establishment is open and operating. Permit is approved. Please address the following items prior to the re-inspection on 2/8: 1. Provide a spill resistant vacuum breaker (SVB) (ASSE 1056) for the hose reel at the dish washing machine. Install the backflow preventer 12” above the water connection point of the hose reel. Certify the backflow preventer and provide report. 2. Provide a drain trough for the tilt kettle. Drain trough can be above ground and indirectly discharge to a floor sink with approved air gap from the finished floor. 3. Provide a rigid drainpipe for the drain from the steamers and route the pipe to the nearest floor sink with approved air gap. 4. Repair/ replace leaking drainpipe under the 3-comp sink. 5. Re-attach the drainpipe from the ice bin. It felt off. Ensure there is approved air gap. 6. Provide cleanout cover where missing. 7. Seal around the fire sprinkler inside the walk-in cooler and provide sprinkler head cover. 8. Repair / replace the wall panels and floor at the share wall of the walk-in cooler and walk-in freezer. There is ice condensation on the floor and the pooling water is damaging the floor and wall of the walk-in cooler side. 9. Provide strainers for floor sinks. 10. Noted one of the floor sink under the cold self-service bar is missing. Per facility engineer, the drainpipe for these units collapsed and currently the facility is not using these units. Please repair/ replace. A remodel application must be applied and approved for construction prior to the repair. 11. Noted one section of the currently un-used service area is missing pre-fab stainless steel work station; therefore, exposed concrete floor, large gaps at wall and curb-up and missing tiles around the curb-up. Please repair and seal all gaps, cracks and penetrations. 12. Repaint/ refinish / replace the damaged walls in the food prep area. 13. Caulk and seal all gaps, cracks, gouges and penetrations throughout. This includes inside the walk-in units, under the hood, walls and hood panels, etc. The inspection right sheet is emailed with the report to the owner. No County legal action will result from this inspection. Please contact the Department to schedule a follow-up inspection at 602.506.6824 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
