New owner inspection walk through with the old and new owner: 1). For the tile throughout repair where tile is broken, loose or missing, please repair behind the front cooler, corners. 2). Throughout where the base tile is open at the top wall juncture please seal, throughout. Use backer rod where too wide. 3). Caulk the under side of the bar at the wall juncture. 4). Please disable the water line to the glass rinser in the beer tap. It is not working and normally a backflow is required. If disconnected no backflow required. 5). In the kitchen, please re-mount the splash guard and silicone the joint. 6). Provide new outlet covers for the behind the large standing fridge at the front and in the kitchen center wall dish was side. 7). Provide new cleanout covers under food prep sink and at the mop sink. 8). Re-mount the three compartment sink and silicone to the back wall. 9). Remove molded caulk at the dish wash gutter and re-caulk. 10). Caulk all holes in the walls throughout. Also at the toasters where the shelve was. 11). Two oil pans are missing from the hood, not too important for the back steam hood but if you can find them great. 12). Raise the dry storage shelve to 6 inches across from the walk-in entrance. 13). Provide sufficient lighting in the walk-in cooler. One can use a bulb replacement. One other one if not fixable replace light with vapor tight fixture. 14). Silicone the mop sink at the FRP trim junctures. 15). See if you can finish the mop sink wall ends with semi-gloss paint or FRP. 16). Replace or repair the atmospheric vacuum breaker on the mop sink faucet. 17). Re-mount the light shield at the mop sink. 18). Provide sufficient light in the restroom stalls. 19). The Backflow valve for the soda system is leaking, please have the valve tested and re-certified. Provide test report. 20). Remount the one loos toilet in the ladies room, shim and caulk at the floor juncture. 21). Please touch up missing grout in the rest rooms at the floor / wall juncture. 22). See if you can attach the rubber leg for the top microwave. 23). Please close the gap under the side door with new or lowered door sweep. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
