Final walk through of the deli with the contractor and sprouts representative. Construction is complete and approved. The following items will be followed up upon by the contractor in the next week or so: 1). Hot water temperature of the water heaters will be set to 140 degrees Fahrenheit. It was currently at 120 which drops down by the time it gets to the three compartment sink. Make sure to adjust all the hand sink tempering valves accordingly. 2). A pray down proportioner was installed next to the three compartment sink which will require a pressure vacuum breaker upstream. Please install backflow device and have the valve tested and certified. 3). The connection to the backflow valve under the three compartment sink is leaking. 4). The hand wash sink drain is leaking. 5). The ice machine connection is leaking. 6). All the toe kicks on the refrigerated cases will be put back in place. 7). Please base out the service counter on the kitchen side. 8). Drain grates will be installed where missing. Also at the end of the meat case all the way east. 9). Where the stainless coved base is not tight with the floor the base will be silconed to the floor. A few spots were missed. This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. It is also the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Permit issued Eating and Drinking Class IV 0-9 Seating.
