FD-86102. Class 3 E&D 10+ Seating. Permit Approved. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are non-refundable. New owner inspection conducted with owner, Ty. Ty acknowledged receipt of inspection rights. Emailed report to Ty. Please address the following prior to re-inspection: 1. Re-route drain line from prep sink to floor sink. Ensure all wall penetrations are sealed off. Ensure a rigid drain line with a 1 inch air gap at floor sink. Currently routed to mop sink. 2. Provide an ASSE 1022 dual check valve for espresso machine. 3. Have ASSE 1013 reduced pressure zone assembly device for carbonated beverage machine tested and certified. Provide certificate. 4. Remove y-valve from mop sink. 5. Provide splash guard on right side of front hand wash sink since clean dish rack is stored within 24 inches. 6. Ensure all drain lines at floor sink maintain a 1 inch air gap. 7. Bathroom door needs to be self closing. 8. Remove fans from kitchen area. 9. Repair ice machine door. It is repaired with tape. Tape is not cleanable. 10. Clean under ice machine. 11. Paint raw wood edges with semi or high gloss paint. Discussed refinishing or changing laminate counter top with solid surface or epoxy. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No County legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
