Inspection Comments

OC inspection for a Class 2 Retail Food Establishment permit was conducted with management by appointment. Inspection rights were delivered via email to Subhash T. The establishment was open and operating at time of inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Provide a 30 gallon minimum capacity water. Observed a 19 gallon capacity unit installed. 2) Provide a separate dedicated handwash sink near the three compartment sink. Provide splash guards. 3) Provide an ASSE 1013 reduced pressure backflow preventer upstream of the soda carbonator. Ensure no copper is downstream of the backflow. Certify and provide a test report. 4) Trim the indirect drain lines at the soda area floor sink to ensure 1" minimum air gaps are provided. 5) Install ASSE 1022 dual checks on each non-carbonated beverage equipment piece. 6) Caulk-in all plumbing penetrations and escutcheon plates throughout the facility with silicone. 7) Re-caulk all sink fixtures to the walls with silicone. 8) Provide an integral cove base (3/8th inch minimum) to the floor/wall and floor/millwork junctures of the restroom and beverage station. 9) Repair the ventilation fan in the restroom. 10) Provide a clear epoxy seal to the laminate beverage counter. 11) Replace the missing cabinetry under the three compartment sink. 12) Provide dome grates at the floor sinks. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
