Final Construction Inspection - Retail Food This inspection was conducted with the on site superintendent. A PDF copy of this inspection report was emailed to Chris H. at the time of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the following prior to your first routine inspection; 1. Remove the extendable sprayer from the sink in the floral area across from Starbucks or provide testable backflow prevention devices for the water supply lines to the sprayer. 2. Seal the space between the Pick-up area cooler and the adjacent wall. 3. Certify the RPZ backflow prevention device for the water supply line to the RO system for the produce mister system. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. Remove the brass fittings on the downstream side of the backflow device in the RO system area. 4. Seal all cement curbs installed on the inside and outside portions of all walk-in units to be waterproof and provide the caulked, cove base finish at the floor/wall junctures. Repair the rough corners of the cement to be smooth, non-absorbent and easily cleanable. 5. Cover and seal all open spaces between walk-in units and adjacent wall and the open spaces between the drywall and the outer portions of the walls around the can wash/mop sink area in the back room. No county legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
