E&D, class 4 10+ Final construction inspection was conducted with the owner, Jesus P. Construction had been completed. The walk-in cooler and walk-in freezer were not installed due to supply chain issue. A minor remodel permit will be created to address the installation of the walk-in units in the future. Permit is approved. Please address the following items before the routine inspection: 1. Provide an additional water stub out for chemical dispenser at the mop sink. Per contractor, the establishment will not use chemical dispenser. 2. Elevate the end of the drainpipes from the roof and the drain pan from the water heater to 1” above the flood rim of the mop sink. 3. Extend the end of drainpipes from 3-comp sink, prep/ work sinks 2 or 3 more inches closer to the floor sink so the force from the water discharge is not splashing around the facility. Ensure minimum 1” air gap is obtained from the end of the drainpipe to the flood rim of the floor sink. 4. Provide self-closer or self-closing hinges on all exterior doors. 5. Connect the ice cream machine. Route the drainpipe to the nearest floor sink with 1” air gap from the finished floor. Ensure drainpipe is rigid. 6. Fill the gap at door frame and floor junctures. 7. Provide threshold at the backdoor to smooth out the transition between the different floor level. 8. Provide door sweep on the backdoor. Emailed the report to the owner. No County legal action will result from this inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.
