Inspection Comments

Final construction inspection. Construction complete and approved. Store is okay to open pending City Approval due to the fact that they will be operating on temporary power. The store will switch off the generator before the end of January. The following items were discussed with the superintendent to follow up on before opening: 1). Install an additional light in the walk-in refrigerator. 2). Plumber is to: a). Install uni-struts for the spray arms at the three compartment sink. b). Water lines to the steamers will be installed, dual check valves are already on site. c). Water leak at the hand sink will be repaired. d). Hub drains will be installed or the reduced pressure zone valves on the soda system and the rethermalizer. e). Hole next to the toilet will be filled in and floor restored to match finish in place. f). Toilets and hand sinks will be caulked at the floor / wall junctures in the restrooms. g). Spray arm is leaking at the three compartment sink. h). Two of the hub drains that will not be used will be capped. 3). Door sweep will be installed at the back door to make the door weather and insect tight. 4). Ceiling tiles that are out of place will be put back into place, those that need to be notched out will be notched out. 5). Second drive through FRP will be trimmed out at the base juncture. This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. It is also the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Permit issued Eating and Drinking Class III 10+ Seating. Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
